MISSION STATEMENT

The mission of the division of Human Resources is to support the Agency, its strategic plan and Agency-wide goals by:

Providing a qualified workforce through effective recruitment and retention of individuals who best provide the necessary skills needed to carry out the mission of the Agency.

Facilitating the work process through the orientation and training of employees in areas of policies and procedures, supervisor and management development and other areas related to the employee’s personal and professional development.

Providing leadership, management information, and data relative to human resource issues and the Agency workforce, which will enhance operations, planning and decision making, as well as meet the reporting requirements of external entities and agencies.

The Agency offers the option of supplemental insurance with AFLAC and Colonial. Also included in the package are leave time, tuition reimbursement, tax-sheltered annuity options, and short-and long-term disability insurance.

Dental and Medical Insurance Highlights

Aetna

StarrVista offers voluntary dental insurance through Aetna. Employees can select the DMO or PPO benefit. Choosing the DMO benefit will enable employees to choose an Aetna Dentist within the DMO network that will allow greater benefits at a lower cost. For greater freedom of choice, employees can elect the PPO benefit, which allows one to select any dentist. For a detailed description of both plans, refer to the Employee Benefit News and/or the Aetna Dental packet.

Care Choices HMO*

Benefits include:

No deductible

Most services covered at 100%

Coverage for preventive health care

$0 office visit co-pay

$0 emergency room co-pay

$5 generic/ $10 brand-name prescription drug co-pay (including contraceptives)

Mail order prescription drug program (two co-pays for a 90 day supply)

Full vision coverage. Exams and lenses are allowed once every 12 months and frames are allowed once every 24 months.

M-Care HMO*

Benefits include:

No deductible

Most services covered at 100%

Coverage for preventive health care

$10 office visit co-pay

$50 emergency room co-pay (waived if admitted)

$10 generic/$15 brand-name prescription drug co-pay (including contraceptives)

Vision coverage: Exams are allowed once every 12 months up to $40. Lenses and frames are allowed once very third year up to $60.

Long Term Disability (LTD)– Prudential Financial

StarrVista provides LTD through Prudential Financial. Features of the plan include:

Pays 60% of your base monthly earnings to a maximum of $5,000 per month

Benefits begin after 90 days of disability

Benefits are payable to age 65 and can be offset by social security, workers compensation, etc.

Benefits are taxable upon receipt.

Life/AD&D Insurance – Prudential Financial

StarrVista also offers paid life insurance to its employees through Prudential Financial.

Life Insurance and Accidental Death and Dismemberment plan features:

1 x annual earnings to a maximum benefit of $50,000

Optional Life Insurance – The Guardian

StarrVista offers an Optional Life Insurance Program through The Guardian. This affords employees the opportunity to sign up for additional term life insurance at very competitive group rates and the cost is conveniently deducted from your paycheck. Each employee may select an additional benefit for themselves, his or her spouse, and eligible dependents.

Pooled Time Off (PTO)*

Pooled time off is calculated as follows:

0-4 years of continued service will accrue up to 120 hours (15 days)

5-10 years of continued service will accrue up to 160 hours (20 days)

11+ years of continued services will accrue up to 200 hours (25 days)

401 (k) Plan**

All full-time employees are eligible to participate in the plan after meeting the following requirements:

Attained the age of 21

Completed 6 months of service

The Plan allows you to contribute money to your Plan account each pay period through convenient payroll deduction. Your salary reduction contributions and all earnings on your account are not subject to current federal income tax (or, where applicable, state or local taxes) until you take them out the Plan.

Employee Assistance Program (EAP)***

The EAP is a company-sponsored benefit provided by T.E.A.M., Total Employee Assistance & Management, Inc., and is available to all eligible employees and their immediate family members at no cost. An Employee Assistance Program or EAP, is what its name says it is: a services that assists employee. The EAP provides confidential assistance for you and your family members to help you resolve any concerns that are affecting your personal or work life.
